The Certified Medical Assistant (CMA) plays a crucial role within the Patient Care Team (PCT), collaborating closely with the Provider both in-home and in other settings. The CMA and Provider work in tandem to ensure optimal care delivery and effective follow-up actions. The CMA operates under the direct supervision of the Provider at all times, adhering to licensure, privileges, and scope of practice regulations.

This position is field-based, requiring travel to patient residences. We are looking for Medical Assistants (CMAs) in various counties.

Key Responsibilities:

  • Assist the Provider in all aspects of patient care before, during, and after home visits, including post-visit office tasks.
  • Engage directly with patients during home visits, which includes:
    • Providing translation services or connecting with language assistance for patients and their families.
    • Gathering and documenting essential information such as demographics, care preferences, medication details, vital signs, and other data as directed by the Provider.
    • Conducting EKGs or other tests as permitted by the Provider.
    • Performing blood draws as instructed by the Provider.
    • Executing various clerical duties, including preparing patient charts for visits.
    • Conducting outreach to schedule home visits for Clover clinical programs.
    • Reviewing and obtaining patient signatures on necessary consent forms during initial and follow-up visits.
  • Remind the Provider of open quality gaps and priority items for in-home visits.
  • Coordinate care with other PCT members using EHR task management software.
  • Handle and prepare blood specimens for laboratory processing, including spinning blood in a vehicle centrifuge and updating patient EMR records as needed.
  • Transport the Provider to patient appointments, ensuring timely visits and managing logistics.
  • Maintain medical supplies for both the CMA and Provider as per practice standards.
  • Ensure the company vehicle is maintained according to the Company Fleet Management Policy.
  • Stay with the Provider throughout each visit.
  • Engage in ongoing professional development and self-improvement.
  • Exhibit flexibility in assignments within Clover's Clinical Programs based on the needs of members and the company.

Qualifications:

  • 0 to 1 year of relevant work experience, preferably in home care.
  • Certification as a Medical Assistant from an accredited program is required.
  • Basic Life Support (BLS) certification must be obtained within 30 days of hire.
  • Fluency in English and Spanish, both spoken and written, is required.
  • A valid driver's license is required.
  • Ability to commute to various locations as needed.
  • Strong organizational skills and the ability to manage multiple tasks efficiently.
  • High School Diploma or equivalent is required.
  • Basic computer proficiency is required; experience with EMR systems is preferred.
  • Excellent interpersonal skills with the ability to communicate effectively with patients and colleagues.
  • Proficient in taking vital signs, drawing blood, updating medical records, preparing patients for examinations, and performing basic laboratory tasks.
  • Comfortable with physical demands, including climbing stairs and carrying equipment.
  • Ability to work with a diverse patient population.
