Job Summary

We are seeking a Senior Patient Care Manager to join our team in Burlington, NC. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice site, ensuring the provision of high-quality care to patients, and managing a team of clinical staff.

Key Responsibilities
  • Oversee the clinical aspects of the hospice program, ensuring compliance with regulatory guidelines and managed care principles.
  • Manage and direct the services and performance of assigned team members, including supervising, managing performance, and directing the work of specified team members.
  • Ensure the timely completion of admit and evaluation visits, scheduling visits, and managing staff workload.
  • Manage the site in the absence of the Executive Director or Administrator, overseeing clinical and administrative functions, and responding to phone referrals and inquiries.
  • Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
Requirements
  • Registered Nurse (RN) with ASN or BSN from an accredited school of nursing, one-year experience as an RN, and two or more years' experience in hospice or home care.
  • Current license to practice nursing in the state of requested employment and CPR Certification.
  • Sensitivity to the feelings of the terminally ill and their families, with the ability to demonstrate flexibility and communicate effectively with people of all socioeconomic backgrounds.
