As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families in the Orange County area.
Job Summary:The Hospice RN Long Term Care Case Manager will be responsible for managing the overall plan of care for a caseload of hospice patients in collaboration with other members of the interdisciplinary group. This includes assessing and identifying patient/caregiver problems, establishing nursing goals, and interventions. You will work closely with physicians to establish medical plans of care and provide emotional support and teaching to patients and caregivers.
Responsibilities:- Manage the overall plan of care for a caseload of hospice patients
- Assess and identify patient/caregiver problems, establish nursing goals, and interventions
- Work closely with physicians to establish medical plans of care
- Provide emotional support and teaching to patients and caregivers
- Communicate effectively with team members, patients, caregivers, physicians, pharmacists, and other healthcare professionals
- Document the outcome of care
- Provide dietary counseling for patient care needs as indicated
- Assume on-call responsibilities as scheduled
- Graduate of an accredited School of Nursing
- Currently licensed as an RN in North Carolina and in good standing with NCBON
- Experience in an acute, hospice, home health setting, or long-term care setting a plus
