Key Responsibilities:

  • Provide guidance, support, and observation to clients participating in community access services.
  • Develop a structured schedule that promotes community inclusion and engaging learning experiences.
  • Facilitate and oversee small group outings and volunteer opportunities.
  • Maintain accurate documentation including goal tracking, attendance, and progress notes.
  • Assist in building natural support networks within the community.
  • Offer vocational training for individuals interested in employment opportunities.
  • Collaborate with community members to conduct hands-on workshops.
  • Support individuals in maintaining health, safety, and daily living activities.
  • Provide transportation using agency vehicles or personal vehicles as needed.
  • Participate in team meetings and various training sessions.
  • Engage in cross-training as required by the organization.
  • Communicate regularly with colleagues regarding client progress and any concerns.
  • Perform additional duties as assigned.

Qualifications:

  • High School Diploma required.
  • Preferred experience in a related field.
  • Must maintain satisfactory performance and standing.
  • Reliable transportation and valid driver's license required.

Physical and Work Environment:

The physical demands of this role include the ability to sit, stand, walk, and occasionally lift up to 40 pounds. The work environment may vary based on location and individual needs.
