Key Responsibilities- Conduct initial case assessments and file reviews to identify youth needs and develop individualized case plans.
- Collaborate with youth, families, and community providers to ensure comprehensive service delivery and coordination.
- Develop and implement case management plans, including goal setting, progress monitoring, and outcome evaluation.
- Provide individual and group counseling services to youth, addressing their emotional, behavioral, and social needs.
- Coordinate and facilitate group services, including education, recreation, and life skills training.
- Monitor and report on youth progress, identifying areas of improvement and developing strategies for success.
- Participate in staff development and training, staying current on best practices in case management and youth services.
- Supervise and mentor junior staff members, providing guidance and support to ensure effective case management practices.
- High school diploma or equivalent, plus 10 years of full-time wage-earning experience in case management or a related field.
- Or, Bachelor's degree in a related field, plus 5 years of experience in case management or a related field.
- Or, Master's degree in a related field, plus 3 years of experience in case management or a related field.
- Must be at least 23 years of age.
- Must pass a pre-employment drug test, fingerprinting, and TB testing.
- Certification as a TBRI Practitioner or Dialectical Behavior Therapy training.
- Completed undergraduate or graduate course hours in a related field.
