Director of Case Management

We are seeking a highly skilled and experienced Director of Case Management to lead our Case Management Department. The successful candidate will be responsible for providing strategic direction, oversight, and support to ensure the delivery of high-quality patient care services.
Key Responsibilities- Develop and implement case management policies, procedures, and protocols to ensure compliance with regulatory requirements and organizational standards.
- Provide leadership and guidance to case management staff, including training, coaching, and performance management.
- Collaborate with healthcare teams to ensure seamless transitions of care and effective communication.
- Monitor and evaluate the effectiveness of case management services, identifying areas for improvement and implementing changes as needed.
- Develop and manage budgets, ensuring efficient use of resources and optimal patient outcomes.
- Current RN license in the state of Florida.
- Master's degree in Nursing required.
- National nursing certification required within six months of hire.
- Minimum of five years professional nursing practice experience required.
- Demonstrated leadership and management skills with knowledge and application of case management, care delivery models, healthcare delivery systems, CQI process, and budgetary process.
- Ability to create vision, effect change, and facilitate transitions.
Employees in this position will operate vehicles for an assigned business purpose as a 'frequent driver'. A frequent driver is defined as one who uses their personal or Shands automobile at least once daily, at least five individual trips per week, or drives over 150 miles per week in the performance of their job.
