Key Responsibilities:
  • Build strong relationships with residents, families, and community partners to ensure seamless support services.
  • Conduct thorough assessments to identify resident needs and preferences, ensuring 95% of residents are contacted within the last 12 months.
  • Develop and facilitate resident activities, support groups, and educational programs to promote socialization and community engagement.
  • Collaborate with internal teams to provide comprehensive support services, including in-home support services, counseling, and advocacy.
  • Model and promote courteous, respectful, and non-confrontational communication skills, adhering to our organization's values and mission.
  • Stay up-to-date with regulatory requirements and best practices in senior services, ensuring compliance and excellence in service delivery.
Requirements:
  • High school diploma or equivalent required; Bachelor's degree in social work, psychology, or related field preferred.
  • Professional Service Coordinator (PSC) designation and experience in affordable housing environments are highly valued.
  • Excellent interpersonal and communication skills, with the ability to work effectively with diverse populations.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple projects.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
