Job Summary

We are seeking a skilled Medical Social Worker to join our team at Sutter Health. As a Medical Social Worker, you will play a vital role in assessing social and emotional factors related to patients' illnesses, medical, and nursing requirements in the home.

Key Responsibilities
  • Assess social and emotional factors related to patients' illnesses, medical, and nursing requirements in the home.
  • Develop and implement a plan for home social work services, including education of patients, family members, and caregivers.
  • Coordinate with other home health staff and community resources to provide an efficient integration of care.
  • Provide education and support to patients, family members, and caregivers to promote healthy behaviors and improve patient outcomes.
  • Maintain accurate and up-to-date records of patient interactions and progress.
Requirements
  • Masters degree from a program accredited by the Council on Social Work Education.
  • Current Basic Life Support (BLS) certification.
  • Valid driver's license and automobile insurance.
  • One year of experience as an MSW in a healthcare setting is strongly preferred.
Preferred Qualifications
  • Knowledge of local community resources.
  • Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury.
  • Ability to maintain harmonious, constructive working relationships with internal and external customers.
Working Conditions

This is a full-time position with a regular schedule. The salary range for this role is $42.90 to $56.89 per hour. Sutter Health offers a comprehensive benefits package and a positive work environment.
