Job Summary

As a Social Worker at SSM Health, you will play a vital role in ensuring patients receive comprehensive care and support during their hospital stay. This position requires a strong understanding of social determinants of health and the ability to develop and maintain working relationships with community health, welfare, and social agencies.

Key Responsibilities
  • Assessment and Intervention
    • Conduct thorough assessments of patients' social needs and develop individualized plans to address these needs.
    • Collaborate with the healthcare team to ensure patients receive timely and effective interventions.
  • Community Resource Referral
    • Identify and connect patients with community resources that can provide support and assistance.
    • Develop and maintain relationships with community agencies to ensure seamless referrals and follow-up care.
  • Documentation and Communication
    • Accurately document patient interactions, assessments, and interventions in the electronic medical record.
    • Communicate effectively with patients, families, and healthcare team members to ensure clear understanding of patient needs and progress.
Requirements
  • Education
    • Master's degree in Social Work (MSW) from an accredited program.
  • Experience
    • No prior experience required, but a strong understanding of social work principles and practices is essential.
  • Skills
    • Excellent communication and interpersonal skills.
    • Ability to work effectively in a fast-paced environment.
    • Strong analytical and problem-solving skills.
