RN - Mechanical Heart Coordinator

We are seeking a highly skilled RN - Mechanical Heart Coordinator to join our team at Providence Sacred Heart Medical Center & Children's Hospital. As a key member of our cardiovascular team, you will play a critical role in coordinating transplant-related activities for patients across the continuum of care.
Responsibilities- Coordinate patient evaluation, waiting list registration, and patient readiness for deceased donor transplants.
- Arrange transplant admission, monitor clinical pathway progression, and provide post-discharge follow-up.
- Provide patient and family education, professional/peer education, and participate in the development of patient education programs.
- Maintain 24-hour on-call coverage for organ offers and other transplant needs.
- Participate in outreach clinics with physicians, recipient selection for placement on the UNOS wait list, and communicate decisions to patients, nephrologists, and dialysis programs.
- Provide inpatient and outpatient clinic transplant education, ongoing education, monitoring of labs, and arrange outpatient infusion and radiologic procedures.
- Communicate and set up initial post-transplant visits with primary transplant physicians upon return to recipients' homes.
- Assist with program development and awareness, research, and other duties as delegated.
- Bachelor's Degree in Nursing.
- Washington Registered Nurse License.
- Remains up to date on UNOS-OPTN and CMS regulations.
- 5 years of Acute care nursing experience.
- Certified Clinic Transplant Coordinator (CCTC) certification.
- 3 years of Current experience in Cardiology nursing, including transplantation.
