Mechanical Heart Case Manager

We are seeking a skilled and compassionate RN to join our team as a Mechanical Heart Program Coordinator. In this critical role, you will provide expert coordination and case management for mechanical heart patients, ensuring seamless care and support throughout their journey.
Key Responsibilities- Coordinate and manage mechanical heart patient care, including ventricular assist device preparation, device readiness, and post-implant analysis.
- Collaborate with multidisciplinary teams to ensure effective patient care and outcomes.
- Develop and implement patient education programs to promote informed decision-making and self-care.
- Provide professional, peer, and patient education to enhance knowledge and skills.
- Participate in the development of patient education programs to support mechanical heart patients.
- Bachelor's Degree in Nursing
- Washington Registered Nurse License upon hire
- 5 years of Acute, Ambulatory or Clinic nursing experience.
- 3 years of recent experience in critical care/transplant nursing.
