About the Role

We are seeking a highly skilled and experienced Cardiovascular Transplant Coordinator to join our team at Providence Sacred Heart Medical Center & Children's Hospital. As a key member of our cardiovascular team, you will play a critical role in the coordination and management of transplant-related activities for patients across the continuum of care.

Key Responsibilities
  • Coordinate and manage transplant-related activities for patients, including pre-transplant, post-transplant, and ongoing care.
  • Provide patient and family education on transplant-related topics, including the transplant process, post-transplant care, and medication management.
  • Coordinate patient evaluations and maintain waiting list registration for deceased donor transplants.
  • Arrange transplant admissions and monitor clinical pathway progression.
  • Provide post-discharge follow-up and ongoing care for patients.
  • Participate in professional and peer education and contribute to the development of patient education programs.
  • Maintain 24-hour on-call coverage for organ offers and other transplant-related needs.
  • Prepare for and participate in outreach clinics with physicians.
  • Participate in recipient selection for placement on the UNOS wait list as part of the transplant committee.
  • Communicate with patients, nephrologists, and dialysis programs regarding transplant decisions and progress.
  • Provide inpatient and outpatient clinic transplant education and ongoing care for patients.
Requirements
  • Bachelor's Degree in Nursing
  • Washington Registered Nurse License
  • Remains up to date on UNOS-OPTN and CMS regulations
  • 5 years of Acute care nursing experience
Preferred Qualifications
  • Certified Clinic Transplant Coordinator (CCTC) certification
  • 3 years Current experience in Cardiology nursing, including transplantation
