Position Overview

The Registered Nurse 1, Care Line, plays a crucial role in delivering telephonic triage and patient care advice for various health concerns. This position involves client counseling, patient advocacy, health education, and resource management for ChenMed patients and their families. The role requires providing remote clinical guidance and assessments while collaborating with primary caregivers to ensure a comprehensive care approach.

Key Responsibilities

  • Engage with patients through phone or video consultations, gathering health histories and documenting essential data in our systems.
  • Conduct health assessments and propose treatment solutions, monitor patient health, and support wellness initiatives.
  • Utilize available technology to monitor vital signs such as blood oxygen levels, heart rate, and blood pressure.
  • Identify patient symptoms through video interactions and analyze data to recommend appropriate health maintenance strategies.
  • Collaborate with on-call providers to achieve optimal clinical outcomes for patients and their families.
  • Document patient progress towards expected outcomes, ensuring clarity and understanding in all communications.
  • Contribute to enhancing the quality and effectiveness of our telehealth services through ongoing evaluation and improvement.
  • Utilize resources effectively to deliver safe and efficient services while maintaining fiscal responsibility.
  • Perform additional duties as assigned by management.

Qualifications

  • Comprehensive knowledge of nursing practices and procedures.
  • Strong commitment to providing exceptional customer service.
  • Ability to maintain current nursing knowledge and adapt to evolving best practices.
  • Excellent communication skills across various formats and technologies.
  • Proven ability to build trust and professional relationships with patients.
  • Strong clinical judgment and problem-solving skills.
  • Ability to prioritize tasks effectively under pressure.
  • Familiarity with Microsoft Office Suite and other relevant systems.
  • Willingness to travel locally for meetings and training as needed.
  • Fluency in English is required; bilingual candidates are preferred.
  • This role requires the exercise of clinical judgment.

Education and Experience Requirements

  • Associate Degree in Nursing is required; Bachelor's Degree is preferred.
  • Nurse Licensure Compact license is required, with potential for additional licenses.
  • Basic Life Support (BLS) certification is required within the first 90 days of employment.
  • A minimum of 2 years of acute clinical nursing experience is preferred.
  • Experience in telehealth is preferred.
