Role Overview
The Registered Nurse 1, Care Line, plays a crucial role in delivering telephonic triage and patient care guidance for various health concerns. This position involves client counseling, patient advocacy, health education, and resource management for ChenMed patients and their families. The incumbent will provide remote clinical assessments and advice, collaborating with primary caregivers to ensure a comprehensive care approach.
Key Responsibilities
- Engage with patients through phone or video consultations, gathering health histories and documenting essential data.
- Conduct health assessments and develop treatment plans, monitoring patient well-being and promoting wellness.
- Utilize available technology to track vital signs and assess patient conditions.
- Analyze patient data to identify health maintenance strategies and expected outcomes.
- Collaborate with on-call providers to support clinical objectives.
- Document patient progress and interventions clearly and effectively.
- Contribute to enhancing the quality of telehealth practices and nursing standards.
- Utilize resources responsibly to ensure safe and effective service delivery.
- Perform additional duties as assigned by management.
Qualifications
- Solid understanding of nursing functions and practices.
- Exceptional customer service skills and professionalism.
- Strong communication abilities across various platforms.
- Proficient in Microsoft Office Suite and other relevant systems.
- Willingness to travel locally for meetings and training as needed.
- Fluency in English; bilingual skills are a plus.
Education and Experience
- Associate Degree in Nursing required; Bachelor's Degree preferred.
- Nurse Licensure Compact license is necessary.
- Basic Life Support (BLS) certification required within the first 90 days.
- Preferred experience includes 2 years in acute clinical nursing and 1 year in telehealth.
