The Registered Nurse 1, Care Line, plays a crucial role in delivering telephonic triage and patient care guidance for various health concerns. This position involves client counseling, patient advocacy, health education, and resource management for ChenMed patients and their families. The incumbent will provide remote clinical advice and assessments while collaborating with primary caregivers and the interdisciplinary care team to ensure a comprehensive approach to patient care.

Key Responsibilities

  • Engage with patients through phone or video consultations, gathering health histories and documenting relevant data using a computerized system.
  • Conduct health assessments and propose treatment options, monitor patient well-being, assist in emergencies, and promote overall wellness.
  • Utilize available technology to track vital signs such as blood oxygen levels, heart rate, and blood pressure.
  • Leverage video communication to assess symptoms and conditions, analyzing data to determine appropriate health maintenance strategies.
  • Collaborate with on-call providers to support clinical outcomes for patients and their families.
  • Document patient progress towards desired outcomes, ensuring clarity and understanding in all communications.
  • Contribute to enhancing the quality and effectiveness of telehealth practices and nursing standards within the organization.
  • Utilize resources effectively to deliver safe, efficient, and financially responsible services.
  • Perform additional duties as assigned by management.

  • Comprehensive knowledge of nursing practices, procedures, and methodologies.

  • Fluency in English is required; bilingual candidates (Spanish/Creole) are preferred.

Education and Experience Requirements

  • Associate Degree in Nursing is required.
  • Bachelor's Degree in Nursing is preferred.
  • Nurse Licensure Compact license is required, with potential for additional licenses in the future.
  • Basic Life Support (BLS) certification is required within the first 90 days of employment.
  • A minimum of 2 years of acute clinical nursing experience is preferred.
  • At least 1 year of telehealth experience is preferred.
