Role Overview

The Advanced Practice Provider (APP) I, Care Line is tasked with delivering diagnostic patient care primarily through virtual consultations via video or telephone. This role is crucial for managing after-hours and weekend clinical inquiries, ensuring comprehensive assessment, diagnosis, treatment, and documentation of patients' health statuses.

Key Responsibilities

  • Conduct virtual assessments of both acute and non-acute clinical issues, performing thorough physical evaluations and documenting patient histories.
  • Analyze patient data to develop and implement effective management plans, including health education and referrals.
  • Evaluate the necessity for immediate nursing interventions and facilitate appropriate patient care.
  • Plan patient care with consideration of cultural backgrounds and support systems, acting as a patient advocate.
  • Collect and analyze health data to determine suitable health maintenance strategies.
  • Collaborate with primary care providers and other medical professionals to ensure adherence to health guidelines.
  • Engage with patients and their families to discuss health outcomes and promote positive health practices.
  • Utilize telemedicine to achieve optimal patient outcomes, collaborating with on-call providers as necessary.
  • Document all assessments and interventions clearly and concisely.
  • Contribute to enhancing telehealth practices through data analysis and program evaluation.
  • Communicate effectively using various tools and technologies to foster professional relationships.
  • Ensure the delivery of safe, effective, and financially responsible services.
  • Provide exceptional customer service to all stakeholders.
  • Participate in clinical rounds, risk management programs, and relevant meetings.
  • Adhere to departmental goals, performance standards, and regulatory compliance.
  • Assist in developing telehealth policies and participate in quality improvement initiatives.
  • Coordinate services with other programs and contribute to the overall growth of the organization.
  • Perform additional duties as assigned by management.

Qualifications and Skills

  • Expertise in business acumen and core job functions.
  • Strong understanding of medical practices to operate independently and collaboratively.
  • Proven track record of achieving clinical performance metrics.
  • Technical proficiency in conducting telemedicine visits per regulations.
  • Excellent clinical judgment and problem-solving abilities.
  • Ability to prioritize tasks and work effectively under pressure.
  • Strong communication and collaboration skills with multidisciplinary teams.
  • Proficient in Microsoft Office Suite and other necessary systems.
  • Availability to work evenings, weekends, and holidays as required.
  • Fluency in English; bilingual skills are a plus.
  • Willingness to travel locally for meetings and training as needed.
  • Minimum of three holidays worked per calendar year.

Education and Experience Requirements

  • Bachelor's degree in Nursing (BSN) and a Master's degree in Nursing required.
  • Board certification by AANP or ANCC is mandatory for Nurse Practitioners.
  • Basic Life Support (BLS) certification required within the first 90 days of employment.
  • Multi-state licensure, including FL and additional states, is necessary.
  • A minimum of three years of acute or primary care clinical experience is required.
  • Preferred experience includes at least two years in telehealth.
