In this position, you will serve as a vital resource in telephonic triage. Your key responsibilities will encompass delivering essential patient care guidance, providing counseling, advocating for patients, educating on health topics, and managing referrals effectively.

- Connect with patients through phone or video consultations to collect health histories and deliver necessary care.
- Conduct health evaluations, monitor patient wellness, and promote overall health.
- Leverage advanced technology to track vital signs and assess patient data for effective treatment strategies.
- Collaborate with a multidisciplinary team of healthcare professionals to ensure high-quality care.
- Accurately document patient progress and interventions.
- Contribute to the advancement of telehealth practices and nursing standards.
- Utilize available resources judiciously for safe and effective service delivery.
- Proven knowledge and proficiency in nursing practices.
- Exceptional communication skills and a commitment to providing outstanding customer service.
- Stay informed about the latest trends and best practices in nursing.
- Ability to build trust and foster professional relationships with patients.
- Strong clinical judgment, problem-solving, and prioritization abilities.
- Familiarity with various communication tools and technologies.
- Willingness to travel occasionally for meetings and training.
- Fluency in English is required; proficiency in Spanish or Creole is a plus.
- Minimum of three holidays worked per year.
- Experience in telehealth and working with elderly patients is advantageous.
- Associate Degree in Nursing (mandatory).
- Bachelor's Degree in Nursing (preferred).
- Nurse Licensure Compact license and BLS certification required.
- At least two years of clinical nursing experience, ideally in urgent or critical care environments.
- A minimum of one year of telehealth experience.
