We are seeking a highly skilled and experienced Nurse Practitioner to join our team at Mount Sinai Hospital in a Cardiothoracic Stepdown role. This is a full-time position working night shifts.
Job Summary:The successful candidate will be responsible for providing high-quality patient care in a fast-paced Cardiothoracic Stepdown environment. This includes assessing patients' health status, developing and implementing individualized care plans, and collaborating with the healthcare team to ensure optimal patient outcomes.
Key Responsibilities:- Assess patients' health status through comprehensive health history and physical examination
- Develop and implement individualized care plans based on patients' needs and health status
- Collaborate with the healthcare team to ensure optimal patient outcomes
- Provide education and counseling to patients and families on health promotion, maintenance, disease prevention, and management of acute/chronic illnesses
- Document findings and plan of care in patients' medical records
- Consult with physicians according to established standards, policies, procedures, and protocols
- Master's degree in Nursing with a focus on Cardiothoracic Nursing
- Current New York State License with registration as a Registered Nurse
- Certification as a Nurse Practitioner with current registration
- Collaborative Practice Agreement with a physician and designated protocols
- Experience in Cardiothoracic Nursing with a focus on Stepdown care
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
