Nurse Practitioner-Cardiovascular Institute-Mount Sinai Hospital
Job Summary:We are seeking a highly skilled and experienced Nurse Practitioner to join our Cardiovascular Institute at Mount Sinai Hospital. The successful candidate will be responsible for providing high-quality patient care, conducting comprehensive health histories and physical examinations, and developing and implementing individualized plans of care.
Key Responsibilities:- Assess the health status of patients and families through comprehensive health histories and physical examinations.
- Develop and implement individualized plans of care, including therapeutic and diagnostic procedures, consultations, and laboratory examinations.
- Prescribe medication regimens and interpret findings to alter the plan of care as needed.
- Educate and counsel patients and families on health promotion, maintenance, disease prevention, and management of acute and chronic illnesses.
- Document findings and plan of care in the patient's medical record.
- Consult with physicians according to established standards, policies, procedures, and protocols.
- Participate in team meetings and conferences to enhance an interdisciplinary approach to care delivery.
- Maintain patient and employee confidentiality in the management of information.
- Master's degree in Nursing or related field.
- Current registration as a registered nurse in New York State.
- Certification as a Nurse Practitioner by a national association via examination.
- Collaborative Practice Agreement with a physician and designated protocols.
- Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Neonatal Advanced Life Support (NALS) certifications as required by specialty.
- Excellent interpersonal and communication skills, with the ability to work effectively in a team environment.
