We are seeking a highly skilled and experienced Nurse Practitioner to join our Cardiothoracic Stepdown Unit at Mount Sinai Hospital. As a key member of our interdisciplinary team, you will provide high-quality patient care, work collaboratively with physicians and other healthcare professionals, and contribute to the development of our unit's clinical practices.
Key Responsibilities:- Assess and manage patients with cardiothoracic conditions, including those requiring intensive care and monitoring.
- Develop and implement individualized care plans, taking into account patients' unique needs and preferences.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Participate in unit-based quality improvement initiatives and contribute to the development of evidence-based clinical practices.
- Provide education and support to patients and their families, promoting health literacy and self-management skills.
- Master's degree in Nursing or related field.
- Current licensure as a Registered Nurse in New York State.
- Certification as a Nurse Practitioner in Cardiothoracic specialty.
- Minimum 2 years of experience as a Nurse Practitioner in an acute care setting.
- Excellent communication and interpersonal skills, with ability to work effectively in a fast-paced environment.
