Clinical Nurse-MedSurge-Stepdown-9E-Mount Sinai Hospital-Per Diem/Nights

The Mount Sinai Health System is seeking a skilled Clinical Nurse to join our team at Mount Sinai Hospital. As a Clinical Nurse, you will provide high-quality patient care to a designated group of patients and their families. Your responsibilities will include delivering patient care based on the principles of Relationship Centered Care, assessing and evaluating patient care needs, and collaborating with the interdisciplinary healthcare team to develop and implement the plan of care.
Responsibilities:- Deliver patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards.
- Assess and evaluate patient care needs and apply critical thinking skills in patient care management.
- Integrate relevant assessment and intervention skills in the delivery of nursing care.
- Collaborate with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
- Prioritize all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate.
- Educate patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease.
- Communicate effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team.
- Manage assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively.
- Graduation from an accredited Nursing program.
- Bachelor's Degree in Nursing (BSN) or Associate's Degree in Nursing (ADN) with Nursing experience.
- Licensed as a registered nurse with current registration in New York State.
- Basic Life Saver (BLS) certification.
