Position Overview

We are seeking a skilled Case Manager for our Supportive Services for Veteran Families (SSVF) initiative within the Shallow Subsidy program. The ideal candidate will possess strong interpersonal abilities and a commitment to assisting veterans in achieving their housing and self-sufficiency goals. This role involves assessing the housing needs of veterans who are currently unhoused or at risk of homelessness, employing a holistic approach to support veteran families in maintaining stability and independence.

Key Responsibilities

  • Conduct in-home assessments to evaluate client progress and needs.
  • Facilitate monthly consultations with clients to develop and track an Individual Service Strategy.
  • Collaborate with the Case Management team to enhance service delivery and address long-term financial stability.
  • Assist with housing placements aligned with client preferences.
  • Provide therapeutic interventions directly to veterans as necessary.
  • Guide clients in developing problem-solving skills and long-term planning.
  • Make referrals to internal programs and external partners as needed.
  • Ensure completion of goals outlined in grant agreements and individual housing plans.
  • Participate in team activities, including home visits and crisis intervention.
  • Support special case management projects as assigned by supervisors.
  • Maintain accurate documentation of services provided and client interactions.
  • Engage with clients and their families in community settings to foster support networks.

Qualifications

  • A college degree or 4 to 6 years of relevant professional experience is required.
  • Knowledge of housing resources and supportive services is essential.
  • Strong computer skills and the ability to manage extensive data.
  • Ability to work effectively with diverse populations in various environments.
  • Capacity to handle confidential information and manage a substantial caseload.

Preferred Skills

  • Experience working with individuals facing mental health challenges is preferred.
  • A valid driver's license and reliable transportation are required.
  • Current auto insurance is necessary.
