Reentry Case Manager

Position Summary
Family and Workforce Centers of America is seeking a dedicated Reentry Case Manager to guide individuals transitioning back into society. This role involves comprehensive support, from initial assessment to long-term follow-up, ensuring participants achieve their educational, career, and personal goals.
Essential Job Functions
- Collaborate with correctional facility staff to identify eligible candidates for the program.
- Partner with individuals to develop personalized Individual Development Plans (IDPs) outlining realistic educational, training, and career aspirations.
- Administer comprehensive assessments, including job readiness evaluations, risk/need assessments, and career interest profilers, to determine appropriate program pathways.
- Leverage local labor market information to guide participants in selecting suitable career paths.
- Coordinate with correctional facilities and community partners to connect individuals with pre-employment training, educational opportunities, skill development programs, or direct job placement aligned with their IDP goals.
- Facilitate access to legal services and essential support systems as needed.
- Collaborate with a multidisciplinary team to ensure participants receive comprehensive support addressing criminogenic needs and fostering success.
- Enroll individuals in industry-recognized training programs, occupational skills development, pre-apprenticeship, or apprenticeship opportunities within 15 days of release, based on their IDP goals and program schedules.
- Provide ongoing case management, career coaching, and support services to participants throughout the follow-up period (up to 12 months post-release).
- Maintain regular contact with participants, documenting progress, referrals, and program information in designated databases and files.
- Collect and analyze performance data to measure program effectiveness and identify areas for improvement.
- Communicate effectively with team members, management, and participants.
Skills and Abilities
- Demonstrated understanding of evidence-based reentry practices.
- 3-5 years of experience in reentry or workforce development programs serving individuals with criminal justice backgrounds.
- Proficiency in utilizing various workforce assessment tools, career development resources, and corrections risk assessment instruments.
- 3-5 years of experience in case management or a related role.
- Ability to assess participant needs accurately and facilitate appropriate program/resource referrals.
- Excellent written and verbal communication skills.
- Meticulous attention to detail and ability to maintain accurate records in databases and files.
