Position Overview
We are seeking a dedicated and skilled Speech Language Pathologist (SLP) to join our home health care division. This role is part-time and operates on a pay-per-visit basis.

Key Responsibilities of the Speech Language Pathologist
- Conducts initial and ongoing comprehensive assessments at designated intervals.
- Collaborates in the creation and assessment of the care plan with patients, their representatives, and caregivers.
- Delivers services as prescribed by the physician in accordance with the care plan.
- Regularly evaluates changes in the patient's condition and adjusts services as necessary, obtaining physician orders for modifications.
- Provides counseling and education to patients and families to support progress towards established health goals.
- Designs and instructs therapeutic activities aimed at enhancing language comprehension and production across various modalities.
- Consults with families and caregivers to facilitate patient progress and discharge planning.
- Communicates effectively with the interdisciplinary healthcare team to ensure coordinated patient care.
- Maintains communication with all physicians involved in the care plan and other relevant healthcare providers.
- Completes and submits accurate and timely documentation of treatment interventions and patient progress in line with professional standards and requirements.
Essential Qualifications for the Role
- Graduate of an accredited Speech Language Pathology program with an active license in Michigan
- Minimum of one year of experience as an SLP within the last five years
- Strong clinical judgment, interpersonal skills, and a compassionate approach
- Previous experience in home health care is preferred
