Key Responsibilities of the Speech Language Pathologist (SLP)

  • Conducts initial and ongoing thorough assessments at designated intervals
  • Collaborates in the creation and assessment of the care plan alongside the patient and their caregivers
  • Delivers services as prescribed by the physician in accordance with the care plan
  • Regularly evaluates changes in the patient's condition and adjusts services as necessary, obtaining revised orders from the physician when needed
  • Provides counseling and education to patients and families to facilitate progress towards established healthcare goals
  • Designs and instructs task-oriented therapeutic activities aimed at enhancing language comprehension and production across various modalities
  • Consults with families and caregivers to support patient progress and discharge planning
  • Maintains effective communication with interdisciplinary healthcare team members to ensure coordinated patient care
  • Engages with all physicians involved in the care plan and other relevant healthcare practitioners
  • Completes and submits timely, accurate documentation of treatments, interventions, and patient progress in line with professional standards and policies

Essential Qualifications for the Speech Language Pathologist (SLP)

  • Graduation from an accredited Speech Language Program and possession of an active SLP license in Michigan
  • A minimum of one year of experience as an SLP within the last five years
  • Demonstrated clinical judgment, strong interpersonal skills, and a compassionate approach
  • Prior experience in Home Health is preferred
