Position Overview

We are currently seeking a qualified Speech Language Pathologist (SLP) to join our dedicated home healthcare team. This role is part-time and operates on a pay-per-visit basis.

Key Responsibilities of the Speech Language Pathologist (SLP)

  • Conduct initial and ongoing comprehensive assessments at designated intervals
  • Collaborate in the formulation and assessment of the care plan alongside the patient and their caregivers
  • Deliver services as prescribed by the physician in accordance with the care plan
  • Regularly evaluate changes in the patient's condition and adjust services as necessary, including re-evaluations and obtaining revised orders from the physician
  • Provide counseling and education to patients and families to facilitate progress towards established goals and address healthcare needs
  • Design and implement task-oriented therapeutic activities aimed at enhancing language comprehension and production across various modalities
  • Advise and consult with families and caregivers to support patient advancement and discharge planning
  • Maintain effective communication with interdisciplinary healthcare team members to ensure coordinated patient care and discharge planning
  • Engage with all physicians involved in the care plan and other relevant healthcare professionals
  • Complete and submit accurate and timely documentation of treatments, interventions, and patient progress in compliance with professional standards and policies

Essential Qualifications for the Speech Language Pathologist (SLP)

  • Graduation from an accredited Speech Language Pathology program and possession of an active SLP license in Michigan
  • A minimum of one year of experience as an SLP within the last five years
  • Strong clinical judgment, excellent interpersonal skills, and a compassionate approach
  • Preferred experience in Home Health settings
