Palliative Care Nurse Navigator

We are seeking a skilled Palliative Care RN Nurse Navigator to join our team at Trinity Health. As a key member of our multidisciplinary team, you will function as an advocate, educator, and facilitator of care, ensuring that all referred patients receive quality and comprehensive services throughout the continuum.
Responsibilities- Conduct comprehensive health assessments of patients, including physical, psychological, social, environmental, financial, and functional status, in collaboration with multidisciplinary team members.
- Coordinate timely patient appointments and develop goals, treatment plans, and provider options in concordance with evidence-based standards and clinical trials.
- Work directly with physician leads to review diagnostic studies, clinical history, and course of treatment to identify the most appropriate next course of care.
- Collaborate with multidisciplinary team members in performance improvement activities, including assessment, planning, implementation, and evaluation of clinical outcomes and processes for patients.
- Use critical thinking and problem-solving skills to analyze and synthesize data and formulate an appropriate case management plan to address the patient's individual and family needs.
- Bachelor's degree in Nursing required, Master's Degree in Nursing preferred.
- Current State of Michigan Registered Nursing License, minimum. CPR or BCLS Certified.
- Certified Nurse in sub-specialty (OCN, CNRN) and other sub-specialty certification, preferred.
- Minimum: Three years of experience in an acute care setting. One year of experience in medical, surgical, or radiation oncology (for oncology specialty).
- Preferred: Case management experience or patient care leadership experience. Five years of experience in subspecialty.
