Position Overview
LTC is in search of dedicated Advanced Practice Providers specializing in Palliative Care to become integral members of our Primary Care Provider team.

Job Responsibilities
  • This role requires in-person engagement.
  • Ensure adherence to palliative care policies and procedures concerning clinical care, safety, infection control, hazardous materials, and risk management.
  • Comprehend and apply the National Hospice and Palliative Care Organization's definition of Palliative Care:
  • Palliative care is centered around the patient and family, optimizing quality of life by anticipating, preventing, and alleviating suffering. It encompasses addressing physical, intellectual, emotional, social, and spiritual needs, facilitating patient autonomy, access to information, and choice.
  • Care is delivered and coordinated by a multidisciplinary team.
  • Collaboration and communication among patients, families, and both palliative and non-palliative healthcare providers regarding care needs are essential.
  • Services may be provided concurrently with or independently of curative or life-prolonging treatments.
  • Support patient and family aspirations for peace and dignity throughout the illness trajectory, during the dying process, and post-mortem.
  • Effectively identify, assess, and address palliative and chronic disease management challenges while delivering high-quality patient care.
  • Standard work hours are Monday through Friday, 8 AM to 5 PM.
Employment Type
  • Full-Time
  • 40-45 hours per week.
Qualifications
  • Experience in Palliative Care is highly preferred.
  • Candidates must possess a valid and unrestricted APP medical license in Michigan.
  • A valid DEA license in the state of Michigan is required.
  • Completion of an accredited U.S. APP certification program is necessary.
Compensation and Benefits
  • Potential earnings exceeding $125,000 annually.
  • Sign-On Bonus available.
  • Productivity-Based Compensation Model (No Cap).
  • Clinical Standards and Quality Measures must be adhered to.
  • 15 PTO days each year.
  • $2,500 Annual CME allowance with 3 PTO days designated for CME attendance.
