We are seeking a highly skilled Palliative Care Nurse Practitioner or Physician Assistant to join our team at Corewell Health in Grand Rapids, MI. As a member of our Home Palliative Care team, you will play a critical role in providing comprehensive, quality care to patients and their families.
Key Responsibilities- Collaborate with interdisciplinary staff to develop and implement patient care plans
- Provide patient visits for palliative care patients, both new and established
- Coordinate care with other healthcare providers and community agencies
- Assess patient needs and develop educational programs to support their care
- Ensure compliance with professional standards, laws, and accreditation requirements
- Master's degree in Nursing or Physician Assistant studies
- Current licensure as a Nurse Practitioner or Physician Assistant in the State of Michigan
- Basic Life Support (BLS) certification
- Neonatal Resuscitation Program (NRP) certification (for Nurse Practitioners)
- Pediatric Advanced Life Support (PALS) certification (for Nurse Practitioners)
