Position Overview
LTC Provider Care is in search of dedicated Advanced Practice Providers specializing in Palliative Care to join our esteemed team of Primary Care Providers.

Job Responsibilities
  • This role requires in-person engagement with patients.
  • Ensure adherence to palliative care protocols and procedures concerning clinical care, safety, infection control, hazardous materials, and risk management.
  • Familiarize yourself with the National Hospice and Palliative Care Organization's definition of Palliative Care, which emphasizes patient and family-centered approaches to optimize quality of life by anticipating, preventing, and alleviating suffering.
  • Participate in an interdisciplinary team that coordinates care and services.
  • Engage in collaborative communication with patients, families, and healthcare providers regarding care needs.
  • Provide services that can be offered concurrently with or independently of curative treatments.
  • Support patient and family aspirations for peace and dignity throughout the illness trajectory, including during the dying process and post-death.
  • Identify, assess, and address palliative and chronic disease management issues while delivering high-quality patient care.
  • Standard working hours are Monday through Friday, from 8 AM to 5 PM.
Employment Type
  • Full-Time
  • 40-45 hours per week.
Qualifications
  • Prior experience in Palliative Care is highly preferred.
  • Must possess a valid and unrestricted APP medical license in Michigan.
  • Must hold a valid DEA license in the state of Michigan.
  • Completion of an accredited U.S. APP certification program is required.
Compensation and Benefits
  • Competitive salary potential exceeding $125K annually.
  • Sign-On Bonus available.
  • Productivity-Based Compensation Model with no cap.
  • Clinical Standards and Quality Measures must be adhered to.
  • 15 PTO days each year.
  • $2,500 Annual CME allowance, plus 3 PTO days for CME attendance.
