About the Role

Duke University Hospital is seeking a highly skilled Clinical Pharmacist to join our team in Adult Medical Oncology. As a key member of our pharmacy team, you will provide exceptional care to adult medical oncology patients, working closely with physicians and other healthcare staff to deliver evidence-based, safe, and effective drug therapy.

Key Responsibilities
  • Partner with physicians and other providers to provide clinical pharmacy services, including rounding, limited order verification, and clinical monitoring.
  • Serve as the medication expert to nurses, physicians, and other healthcare staff, providing drug information and ensuring the optimum use of medications.
  • Provide education to patients and their families on medication management and other relevant topics.
  • Develop and implement comprehensive supportive care and medication therapy management plans.
  • Collaborate with ambulatory colleagues to optimize patient transitions of care.
  • Maintain outstanding skills as a clinical pharmacy practitioner and consistently demonstrate the value of pharmacy services.
  • Educate medical, nursing, and pharmacy staff about the management, prescribing, and monitoring of therapies used in patient care areas.
  • Serve as a resource to the Department of Pharmacy for areas of expertise.
  • Participate in research, precepting PharmD students, PGY-1 residents, and PGY-2 residents, and involvement in oncology-related committees.
Requirements
  • Doctor of Pharmacy Degree or BS Pharmacy Degree.
  • With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required.
  • With Doctor of Pharmacy degree, no experience is required.
  • 2+ years of practice experience preferred.
  • PGY1 or Pharmacy Practice Residency preferred.
  • PGY2 in oncology, Internal Medicine, or Palliative Care or equivalent experience preferred.
  • Licensed Pharmacist or eligible to practice pharmacy in North Carolina.
  • Hold or Achieve Board Certification in Oncology Pharmacy or Pharmacotherapy within one year of hire.
