Key Responsibilities:
As an MDS Care Coordinator, you will play a vital role in enhancing the lives of our residents by:

  • Providing compassionate care and support to residents
  • Facilitating the onboarding process for new team members
  • Coordinating resident assessments using RAI tools
  • Acting as an advocate for residents through the interdisciplinary care planning process
  • Managing resident assessments for case mix reimbursement and ensuring compliance with MDS reporting guidelines
  • Participating in Quality Improvement committees to promote positive resident outcomes

Qualifications:
The ideal candidate will possess:

  • A degree from an accredited School of Nursing, preferably a BSN
  • A minimum of one year of nursing management experience in long-term care
  • Two years of professional nursing experience in relevant settings
  • Knowledge of the Resident Assessment Instrument and interdisciplinary care planning
  • Familiarity with OBRA regulations and state reimbursement guidelines
  • Understanding of current laws and regulations affecting nursing care
