Job Overview

Role Objective

The primary objective of this position is to ensure thorough evaluation, management, and care planning for participants. This encompasses conducting initial, semi-annual, and annual assessments to track and support participants' progress. The role also involves the maintenance and management of physical therapy equipment, ensuring it remains in excellent working condition. Furthermore, the position requires the development of effective care and discharge plans tailored to the unique needs of each participant, alongside active participation in Interdisciplinary Team (IDT) meetings to foster cohesive and coordinated care strategies.

Key Responsibilities

  • Conducts initial and annual assessments as assigned and provides suitable recommendations for services.
  • Collaborates as a member of the interdisciplinary team, maintaining regular attendance and participation in team meetings; contributes to care planning and service allocation decisions.
  • Completes evaluations as necessary and formulates a comprehensive plan of care. Evaluations include, but are not limited to, gait analysis, balance assessment, bed mobility, functional status, physical condition, home safety evaluations, restraint assessments, and wheelchair requirements.
  • Engages in care plan formulation and discharge planning for participants, including coordination with other facilities serving PACE members.
  • Offers regular updates to the team regarding participants' therapy status, including discharge plans.
  • Administers treatment to participants and implements physical therapy procedures aimed at achieving established goals, adjusting activities as needed.
  • Evaluates the necessity for adaptive and/or durable medical equipment (DME), making recommendations to the interdisciplinary team in accordance with policy and assisting with the issuance of DME as required.
  • In collaboration with primary care and the interdisciplinary team, assesses the need for specialized footwear and prosthetics according to policy. Evaluates newly issued orthotic shoes, braces, and prosthetics for suitability and proper fit as indicated.
  • Performs minor repairs on equipment as directed by competency requirements and manages equipment inventories.
  • May evaluate the need for ongoing skilled services and authorize physical therapies provided in nursing homes and with contracted agencies or hospitals, following initial evaluation orders from primary care.
  • Develops specialized programs based on participant needs.
  • Participates in committee meetings as necessary.
  • Provides physical therapy coverage for other PACE sites as required.
  • Undertakes additional duties as assigned.

Qualifications

Education:

  • Bachelor's Degree in Physical Therapy from an accredited institution and a current California PT license are required.
  • Completion of a physical therapy program accredited by the American Physical Therapy Association is essential.
  • Current CPR certification is preferred.

Experience:

  • A minimum of one year of personal, clinical/internship, or employment experience providing care or services for adults with disabilities or the frail elderly population is required.

Skills and Knowledge:

  • Strong written and verbal communication skills.
  • Proven ability to problem-solve and exercise sound judgment.
  • Ability to work independently.
  • Proficient in computer skills relevant to the workplace.
  • Demonstrates necessary skills and knowledge as outlined in the position-specific Competency Assessment Profile.

Additional Requirements:

  • Physical strength is necessary to perform job duties.
  • Routine travel between PACE sites and within the community is required.
  • A valid driver's license is mandatory.

Physical Demands:

  • Must be able to remain stationary for 50% of the time.
  • Ability to occasionally move within the office to access files and equipment.
  • Capable of operating a computer and other office machinery, such as calculators and printers.
  • Able to position oneself to maintain files in cabinets, which may involve reaching, kneeling, or crouching.
  • The ability to communicate effectively and exchange information is crucial for this role.
