Job Description:

Constellation Health is seeking a compassionate and empathetic Palliative Care Nurse Practitioner to join our team. As a Palliative Care Nurse Practitioner, you will provide high-quality patient care in a variety of settings, including patient homes and facilities in the area.

Responsibilities:
  • Conduct comprehensive assessments and coordinate care plans with interdisciplinary teams.
  • Provide skilled nursing care, including medication administration and treatments as prescribed.
  • Participate in weekly Interdisciplinary Team Meetings to discuss patient updates and care plans.
  • Maintain accurate and timely documentation in compliance with agency standards.
  • Collaborate with institutions to promote continuity of care.
Qualifications:
  • Master's degree in nursing from a university accredited by the Commission on Collegiate Nursing Education (CCNE) or by the National League for Nursing Accrediting Commission (NLNAC).
  • Current license to practice nursing in the state where the hospice program operates.
  • Current National Board Certification from ANCC or AANP as a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP).
  • Experience in hospice or palliative care, clinical oncology, geriatrics, intensive care, or community and family health preferred.
