About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Palliative Care and Hospice team at Compassus. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families facing chronic, complex, and life-threatening conditions.

Key Responsibilities
  • Patient Assessment and Care Planning: Conduct thorough assessments of patients' physical, emotional, and social needs, and develop personalized care plans to address their unique needs and goals.
  • Collaboration with Interdisciplinary Team: Work closely with our team of healthcare professionals, including physicians, nurses, social workers, and chaplains, to provide comprehensive care and support to patients and their families.
  • Palliative Care and Hospice Services: Provide routine and emergency assessments, recertification visits, and educational services to patients and their families, with a focus on symptom management, pain control, and quality of life.
  • Communication and Education: Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and provide education on disease management, treatment options, and advance care planning.
  • Documentation and Reporting: Maintain accurate and timely documentation of patient care, including assessments, treatment plans, and progress notes, and submit required reports to ensure compliance with regulatory requirements.
Requirements
  • Education and Experience: Master's degree in Nursing required; minimum of 5 years of nursing experience, with a focus on palliative care and hospice services.
  • Certifications and Licenses: Active and unencumbered CNS or Nurse Practitioner license in state of employment required; certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required.
  • Skills and Abilities: Strong communication and interpersonal skills; ability to work effectively in a team environment; strong organizational and time management skills; ability to maintain confidentiality and handle sensitive information.
