Job Summary

Constellation Health Services is seeking a highly skilled Palliative Care Nurse Practitioner to join our interdisciplinary team. As a key member of our palliative/hospice program, you will provide compassionate and empathetic care to patients in their homes and various facilities in the area.

Key Responsibilities
  • Comprehensive Patient Assessment: Conduct thorough assessments of patients' physical and emotional needs, coordinating care plans with healthcare teams and family members.
  • Clinical Expertise: Provide skilled nursing care, including medication administration and treatment, as prescribed by physicians, to patients in their home settings.
  • Interdisciplinary Collaboration: Participate in weekly team meetings, sharing updates on patients' physical, emotional, and spiritual status.
  • Documentation and Compliance: Maintain accurate and timely documentation, adhering to agency standards and regulations.
  • Liaison and Community Engagement: Serve as a liaison with healthcare institutions, promoting continuity of care, and participate in community activities representing Constellation Health Services.
Requirements
  • Education and Certification: Master's degree, post-masters certificate, or doctoral degree in nursing from a CCNE-accredited university, along with current National Board Certification as a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP).
  • Experience and Qualifications: Preference given to candidates with experience in hospice or palliative care, clinical oncology, geriatrics, intensive care, or community and family health, as well as palliative care certification (NBCHPN) or hospice certification (ACHPN).
