We are seeking a highly skilled and compassionate Care Coordinator Specialist to join our team at Comprehensive Community Action. As a Care Coordinator Specialist, you will play a critical role in providing case management and care coordination services to children, adolescents, and families.
About the Role:
- Provide comprehensive assessments on assigned clients on caseload.
- Develop a service plan with clients in accordance with program provisions.
- Conduct home visitations, community outreach, and school-based interventions as needed.
- Ensure accurate and timely documentation of client progress and services provided.
- Maintain confidentiality and adhere to HIPAA guidelines.
- Collaborate with interdisciplinary teams to ensure seamless delivery of services.
- Attend regular supervision and training sessions to enhance professional growth.
Requirements:
- Bachelor's degree in Social Work or related field required.
- Case management experience and certification preferred.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- A valid driver's license and reliable transportation.
Estimated Salary: $55,000 - $65,000 per year
