Care Coordinator for Family Services

We are seeking a highly motivated and compassionate Care Coordinator to join our team at Comprehensive Community Action. The successful candidate will have the opportunity to make a meaningful difference in the lives of children, adolescents, and families.
About the RoleThis is a full-time position that requires flexibility with scheduling to accommodate the needs of our clients. As a Care Coordinator, you will be responsible for providing case management and care coordination services to our clients. This may involve conducting comprehensive assessments, developing service plans, and providing community, school, and home visitation.
Key Responsibilities- Conducting thorough assessments of client needs and developing personalized service plans.
- Providing instruction in adult daily living skills, including behavior management, stress management, and parenting.
- Completing emergency funding, housing, utility assistance, medical, and state benefits applications as needed.
- Maintaining accurate and up-to-date records for client files, adhering to standards set by Medicaid, accreditation, and other relevant regulatory bodies.
- Collaborating with other programs and administration to ensure seamless coordination of efforts and resources.
To succeed in this role, you must possess a BSW/BA or AA degree, along with relevant experience in case management. A case management certificate or equivalent certification is also required. Additionally, you should have demonstrated proficiency in operating an automobile and maintaining a valid driver's license.
