Alliance Health at Marina Bay is seeking a highly skilled MDS Nurse Coordinator to join our team. As a key member of our clinical operations team, you will play a critical role in ensuring the delivery of high-quality patient care.
Key Responsibilities:
- Coordinate the day-to-day functions for all departments regarding MDS and care plans in accordance with current rules and regulations that govern long-term care facilities.
- Ensure completion of assigned initial, quarterly, annual, and change of status MDS and electronically submit them with accuracy and on a time schedule mandated by the Department of Public Health and Medicare.
- Review resident care plans for appropriate goals, problems, and approaches, and ensure revisions as necessary per change in the resident's orders, treatment plan, or condition.
- Collaborate with interdisciplinary teams to develop and implement effective care plans that meet the unique needs of each resident.
- Ensure compliance with all relevant laws, regulations, and accreditation standards.
Requirements:
- One to two years of nursing experience required.
- One year of long-term or sub-acute care experience preferred.
- Current Massachusetts RN or LPN licensure.
