Job Summary

We are seeking a skilled Nurse Coordinator to join our team at Tufts Medical Center. This role will focus on providing exceptional patient care and coordinating services to ensure seamless transitions between departments.

Key Responsibilities
  • Utilize the nursing process to deliver and manage patient care, demonstrating diagnostic reasoning and critical thinking.
  • Inform patients and families of services, provide information on disease processes, tests, and schedules, and offer patient education materials.
  • Work with physicians and their staff to coordinate patient schedules and triage phone calls from patients to assist them and their families.
  • Track patients throughout the continuum of care and follow up with them as needed to check on their status and refer them to resources.
  • Consult and refer patients to other resources, such as LCSW, Oncology Nurse, and Dietician.
  • Serve as a patient and family advocate, involving them in decision-making related to the patient's plan of care.
  • Implement interventions consistent with hospital standards of practice and prescribed medical plans of care.
  • Document the plan of care and meet all unit documentation and complete all patient charges promptly and correctly.
Requirements
  • High school diploma or equivalent.
  • Graduation from an accredited practical nursing program.
  • Licensed Practical Nurse (LPN).
  • Two (2) years of relevant nursing experience.
Preferred Qualifications
  • Three (3) years of relevant nursing experience.
Physical Requirements
  • Prolonged, extensive, or considerable standing/walking.
  • Lifts, positions, pushes, and/or transfers patients and equipment.
  • Considerable reaching, stooping, bending, kneeling, crouching.
  • Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise, and possible exposure to radiation, lasers, electric shock, etc.
  • Regularly exposed to the risk of bloodborne diseases and other transmissible infections.
  • Contact with patients under wide variety of circumstances.
  • Subject to varying and unpredictable situations.
  • Handle emergency and crisis situations.
  • Subjected to irregular hours.
  • May have contact with hazardous materials.
Skills & Abilities
  • Ability to effectively communicate with patients, families, physicians, and healthcare team.
  • Knowledge and application of the nursing process to implement a nursing plan of care.
  • Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages, and cultural patterns in each step of the care process.
  • Skills and knowledge to provide care to the age groups of the population served, including knowledge of growth and development, ability to interpret age-specific data and response to care, and provide age-appropriate communication.
  • Knowledge of pharmacology/medications in administering drugs and monitoring the patient's response.
