Key Responsibilities:
- Provide comprehensive and holistic assessments to identify patient needs and develop individualized care plans.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Implement evidence-based practices to promote patient safety and quality of care.
- Supervise and mentor other nursing staff to ensure consistent delivery of high-quality care.
- Participate in performance improvement initiatives to enhance patient outcomes and staff satisfaction.
Requirements:
- Current licensure as a Registered Nurse in the State of Maine.
- 1 year of clinical experience in hospice care or a related field.
- Associate's or Bachelor's degree in Nursing.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
Working Conditions:
- Alternate shift schedules, including evenings, nights, and weekends.
- Prolonged periods of standing, walking, and sitting.
- Potential exposure to hazardous materials and noxious odors.
- Ability to lift, move, and load 30-50 pounds.
