We are seeking a full-time opportunity for a Board Eligible/Board Certified physician in Internal Medicine, Family Medicine, or Geriatrics to become a vital part of our post-acute care team.

This role offers a structured schedule from Monday to Friday, typically from 9 am to 5 pm, ensuring a harmonious work/life balance along with benefits such as paid time off and holidays.

JOB OVERVIEW

Our dedicated Post-Acute team delivers comprehensive medical services across various healthcare settings, including Skilled Nursing, Rehabilitation, Long Term Care, and Assisted Living facilities. This position is designed to promote an excellent work/life balance.

The Attending Physician will play a crucial role in managing health issues and ensuring a high standard of care for our patients.


Key Responsibilities:


- Conduct thorough examinations and develop effective treatment plans.
- Perform History & Physical assessments for all new admissions to skilled nursing facilities.
- Follow established policies for overseeing patient care and reviewing clinical notes.
- Facilitate patient transitions within and between healthcare environments, including admissions, transfers, and discharges.
- Collaborate with a multidisciplinary team of healthcare professionals.
- Supervise Advanced Practice Providers (APPs) as needed.
- Ensure compliance with policies regarding patient visit frequency.
- Accurately submit charges for all professional services rendered.
- Manage an average caseload of 15 patients per day.

QUALIFICATIONS

The ideal candidate will preferably have completed a geriatric fellowship or possess board certification in geriatrics. However, we welcome applications from any physician with relevant geriatric experience or a willingness to work in a post-acute environment.

- Board eligible/certified in Internal Medicine or Family Medicine.
- Exceptional verbal and written communication skills are essential for effective interaction with healthcare professionals, colleagues, families, and patients.
- A team-oriented approach to patient care is vital.
