We are seeking a full-time, board eligible/certified physician in Internal Medicine, Family Medicine, or Geriatrics to become an integral part of our post-acute care team. This role offers a structured schedule, typically Monday through Friday, promoting a healthy work-life balance.
Key Responsibilities:
- Conduct thorough examinations and develop comprehensive treatment plans.
- Perform initial assessments for all new patients admitted to skilled nursing facilities.
- Ensure adherence to established policies regarding patient care oversight and documentation.
- Facilitate smooth transitions for patients within various healthcare environments, including admissions, transfers, and discharges.
- Supervise Advanced Practice Providers as necessary.
- Maintain compliance with patient visit frequency guidelines.
Qualifications:
- Board eligible or certified in Internal Medicine or Family Medicine.
- Exceptional verbal and written communication skills, essential for effective collaboration with healthcare professionals, colleagues, and patients.
- Commitment to the core values of quality care, teamwork, and personal connections.
- Strong ethical standards and compliance with healthcare regulations.
- Proficient in various electronic medical record systems.
- Active medical license in the state, along with DEA and state CSR certifications.
