Position Overview
We are seeking a full-time, board eligible/certified physician in Internal Medicine, Family Medicine, or Geriatrics to become a vital part of our post-acute care team. This role offers a structured schedule from Monday to Friday, promoting a healthy work-life balance.
Key Responsibilities:
- Conduct thorough examinations and develop comprehensive treatment plans.
- Perform initial assessments for all new admissions to skilled nursing facilities.
- Ensure adherence to established protocols for overseeing patient care and reviewing medical documentation.
- Facilitate smooth transitions for patients across various healthcare settings, including admissions, transfers, and discharges.
- Provide supervision to Advanced Practice Providers as necessary.
- Maintain compliance with policies regarding patient visit frequency.
Qualifications:
- Board eligible or certified in Internal Medicine or Family Medicine.
- Exceptional verbal and written communication skills, essential for collaboration with healthcare professionals, colleagues, and patients.
- Commitment to the core values of quality care, teamwork, and ethical standards.
- Proficient in using electronic medical record systems.
- Possession of an active state medical license and DEA registration.
