Job Title: Physical Therapist Home Health Specialist

Pinnacle Home Care Inc. is seeking a skilled Physical Therapist to join our team of dedicated healthcare professionals. As a Physical Therapist Home Health Specialist, you will be responsible for providing high-quality physical therapy services to patients in their own homes.

Job Summary:

The Physical Therapist Home Health Specialist will assess patients' conditions, develop personalized treatment plans, and deliver therapy to help improve mobility, reduce pain, and restore function. This role involves traveling to patients' homes and adapting treatment plans to suit home environments while ensuring high-quality care and patient safety.

Key Responsibilities:
  • Conduct initial and ongoing comprehensive assessments of clients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Establish treatment plans in consultation with physicians and revise them as necessary.
  • Assist in developing and implementing interdisciplinary client care plans as they relate to physical therapy.
  • Treat clients to relieve pain, develop or restore functional abilities, and maintain maximum performance using physical means such as exercise, massage, heat, light, and electricity.
  • Teach, supervise, and counsel clients and their families in physical therapy programs, instructing them in care and use of adaptive devices.
  • Record evaluation data, treatments, and clients' responses to therapy interventions on a timely basis.
  • Prepare evaluations of clients' conditions and perform periodic reassessments.
  • Supervise Physical Therapist Assistants at least every 30 days.
  • Participate in case conferences, physical therapy staff meetings, agency activities, and in-service programs as necessary.
Requirements:
  • Ability to drive and access clients' homes, which may not be routinely wheelchair accessible.
  • Hearing, eyesight, and physical dexterity sufficient to perform physical assessments and client care.
  • On occasion, may be required to bend, stoop, reach, and move client weight up to 250 pounds, lift and/or carry up to 30 pounds.
  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Licensed or registered by the State.
  • At least one year of institutional experience.
  • Physical Therapist in charge of PT services must have home care experience and at least one year of supervisory experience (two years preferred).
