Physical Therapist

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Physical Therapist to join our team of skilled home health care providers. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in their own homes.
Key Responsibilities:- Assess patients' conditions and develop personalized treatment plans to improve mobility, reduce pain, and restore function.
- Deliver therapy services to patients in their homes, adapting treatment plans to suit home environments while ensuring high-quality care and patient safety.
- Assist in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
- Treat clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means such as exercise, massage, heat, light, and electricity.
- Teach, supervise, and counsel the family and client in the physical therapy program, instructing them in the care and use of adaptive devices such as wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
- Record all evaluation data, treatments, and client's response to therapy intervention on a timely basis, as well as quality assurance data as directed.
- Participate in case conferences, physical therapy staff meetings, agency activities, and in-service programs as necessary, and participate with staff, clients, and physicians in discharge planning activities and complete a Physical Therapy Discharge Summary.
- Supervise the home health aide every 14 days when physical therapy is the only professional discipline involved in the client's care.
- Evaluate the client, the client's significant other(s), and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the client, and what other Agency and community services will be required.
- Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
- The Physical Therapist in charge of PT services must have a home care experience and at least one year of supervisory experience.
- Must be licensed as a Physical Therapist (PT) and hold a Basic Life Support (BLS) certification.
