Pinnacle Home Care Inc. is seeking a skilled Physical Therapist to join our team of dedicated healthcare professionals. As a Physical Therapist - Home Health Care Specialist, you will be responsible for providing high-quality physical therapy services to patients in their own homes.
Job Summary:The ideal candidate will have a strong commitment to delivering exceptional patient care and a passion for working in a home health setting. You will work closely with our interdisciplinary team to develop and implement personalized treatment plans that meet the unique needs of each patient.
Responsibilities:- Conduct comprehensive assessments of patients' conditions and develop treatment plans in consultation with physicians.
- Provide physical therapy services to patients in their homes, adapting treatment plans to suit individual needs and environments.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Document patient progress and outcomes, ensuring accurate and timely reporting.
- Participate in quality improvement initiatives and contribute to the development of best practices in home health physical therapy.
- Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
- Licensed or registered by the State.
- At least one year of institutional experience, with a preference for home care experience and supervisory experience.
- Ability to drive and access clients' homes, which may not be routinely wheelchair accessible.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
