Position Overview:
As a Case Management Specialist I, you will deliver essential case management services in alignment with agency protocols, state regulations, and federal laws.

Your role will involve conducting comprehensive family, safety, and risk evaluations, facilitating the collection of assessment data, and collaboratively formulating treatment strategies with families and service providers.

You will ensure that decision-making is shared, family engagement is prioritized, and cases progress towards resolution.

Key Responsibilities:
1. Deliver case management services in accordance with established guidelines and legal requirements.
2. Conduct thorough family, safety, and risk assessments.
3. Collaborate effectively to create and monitor treatment plans with families and service providers.
4. Utilize solution-focused methodologies to reduce risks, enhance safety, and expedite case resolution.
5. Prepare detailed reports and maintain comprehensive case documentation.
6. Conduct home visits, participate in community meetings, and attend court hearings as necessary.
7. Provide testimony in court when required.
8. Authorize and coordinate necessary supportive services.
9. Refer families to community resources and work in partnership with allied agencies.

In this role, you will advocate for families facing crises or those affected by trauma, substance abuse, mental health issues, domestic violence, multi-generational poverty, child abuse, neglect, and other challenges.

Clear and effective oral and written communication is essential. Additional duties may be assigned as needed.

Specialized Assessment Responsibilities:
1. Respond to the Child Protection Hotline, ensuring thorough and accurate referrals through enhanced screening techniques.
2. Conduct assessments related to fatalities, near fatalities, egregious cases, and institutional abuse.
3. Provide training to various stakeholders on identifying and reporting child abuse and neglect.
4. Represent Child Welfare at community interdisciplinary meetings.

Assessment Caseworker Duties:
1. Perform initial assessments for assigned abuse and neglect referrals.
2. Engage with families to ensure the safety of children within the home.
3. Facilitate the removal of children from unsafe environments when necessary.

Permanency Caseworker Functions:
1. Collaborate with families, service providers, and the judicial system to secure permanency for children through reunification, allocation, guardianship, or adoption.
2. Monitor treatment plan progress and identify obstacles.

Prevention Caseworker Role:
1. Work with families, service providers, and the community in non-court cases to mitigate risks and maintain child safety.
2. Identify and address barriers to progress through intensive in-home engagement.

Adolescent Permanency Caseworker Tasks:
1. Manage cases primarily under the juvenile delinquency court system.
2. Oversee youth placed in Residential Treatment facilities.

Youth in Transition Caseworker Responsibilities:
1. Complete training on engaging youth in a coaching manner and apply these skills in all cases.
2. Assist youth with voluntary cases in finding housing.
3. Support youth in developing independent living skills.

Chafee Program Coordinator Duties:
1. Aid youth in acquiring independent living skills.
2. Promote participation in the Chafee program among youth and the agency.

Facilitator/Diligent Search Functions:
1. Implement concepts from various family engagement models, such as Family Group Conferencing and Team Decision Making.
2. Conduct Permanency Roundtable meetings for identified youth.
3. Perform diligent searches to identify kinship supports and potential placement options.

Professional Development Coach Responsibilities:
1. Develop and deliver group coaching sessions and training programs.
2. Conduct individual coaching sessions using the co-active coaching model.
3. Oversee and enhance the mentorship program.
4. Support staff professional development by creating and monitoring structured learning plans.

Adoption Caseworker Duties:
1. Collaborate with permanency case managers to facilitate concurrent case planning.
