Key Responsibilities- Provide Patient-Centered Care: Utilize the nursing process to assess, plan, implement, and evaluate patient care, ensuring that each individual receives personalized attention and support.
- Collaborate with Healthcare Team: Work closely with physicians, nurses, and other healthcare professionals to coordinate patient care and ensure seamless transitions between departments.
- Develop and Implement Care Plans: Create and implement comprehensive care plans that address the unique needs and goals of each patient, taking into account their medical history, lifestyle, and personal preferences.
- Conduct Health Education and Promotion: Educate patients and their families on healthy habits, disease prevention, and management, empowering them to take an active role in their healthcare.
- Participate in Quality Improvement Initiatives: Contribute to ongoing quality improvement efforts, identifying areas for improvement and implementing evidence-based practices to enhance patient outcomes and satisfaction.
- Communicate Effectively: Maintain open and transparent communication with patients, families, and healthcare team members, ensuring that all parties are informed and engaged in the care process.
- Education: Graduate of an accredited nursing program.
- Experience: Minimum two years of RN experience within the last five years, or successful completion of the Kaiser Permanente NW Ambulatory RN New Grad/transition to practice program.
- Licensure: Registered Nurse License (Washington) within six months of hire, or Compact License: Registered Nurse within six months of hire.
- Certifications: Basic Life Support required at hire, Pediatric Advanced Emergency Assessment, Recognition and Stabilization within six months of hire.
- Previous Experience: Previous experience with population care/case management, triage, and advice.
- Education: Current or future bachelor's degree in nursing or related field.
