Job Title: Community Registered Nurse

Acadian Health is seeking a skilled and compassionate Community Registered Nurse to join our team. As a Community Registered Nurse, you will play a vital role in providing high-quality patient care and promoting health and wellness in our community.

Job Summary:

The Community Registered Nurse will be responsible for providing comprehensive care to patients in their homes and communities. This includes conducting environmental assessments, performing clinical assessments, and developing and implementing individualized care plans. The successful candidate will also be responsible for facilitating continuity of care by interacting with other members of the interdisciplinary care team, including physicians, nurse practitioners, and social services.

Key Responsibilities:
  • Conduct environmental assessments to identify potential health risks and develop strategies to mitigate them
  • Perform clinical assessments to identify patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Facilitate continuity of care by interacting with other members of the interdisciplinary care team
  • Participate in medical research and treatment modalities as directed by the Medical Director
  • Perform other duties as assigned by the Acadian Health Operations Director or Medical Director
Requirements:
  • Current BLS and ACLS certifications
  • Required licensure to perform as a Registered Nurse within the state of employment
  • Ability to obtain Community Paramedicine (CP-C) certification or licensure in the state of employment within one year of hire date
  • Effective verbal, written, and interpersonal communication skills
  • Demonstrated ability to provide effective coaching and leadership
  • Demonstrated teaching and educational facilitation skills
  • Ability to adhere to established standards for educational quality
  • Familiarity with basic computer applications and functions
  • Team-oriented and able to work effectively and efficiently with others
Preferred Requirements:
  • Degree from an accredited college or university
  • Previous trainer or instructor experience
  • Previous Critical Care experience
Interpersonal Requirements:
  • Maintain social, ethical, and organizational norms
  • Ability to accomplish all task details, no matter how small
  • Maintain good personal motivation; develop a sense of ownership of job tasks and results
  • Ability to work under the pressure of meeting strict deadlines
  • Excellent interpersonal and communication skills
Training Requirements:
  • Orientation to job requirements
  • Completion of the Acadian Health clinical education program
  • Completion of ride along shifts with current Acadian Health staff
  • Clearance to perform issued by an Acadian Health Operations Supervisor
  • Obtain Community Paramedicine certification or licensure in the state of employment
  • Participation in the Acadian Health continuing education program
